Chapter 52 - Surgery

Konoha Prison.

The Police Department is responsible for ensuring the safety and security of Konoha. The Uchiha clan oversees the supervision of the village's three major prisons.

Outside the regular cells, two elite Uchiha guards stood vigilant, carefully observing their surroundings. Even beyond the prison walls, additional guards were ready to respond to any disturbance.

Surprisingly, beneath the bright, clean cellblock lay a hidden chamber. The room smelled of strong disinfectant, and the operating table bustled with activity.

Outside the operating room, Uchiha Kaien narrowed his eyes and honed his lightning chakra nature by manipulating a leaf between his fingers. The floor was littered with hundreds of burnt, ashen leaves.

"Kaien‑niisan."

Uchiha Shisui emerged from the op room after escorting a prisoner—but before he could step forward, Kaien shoved him against the wall, and Shisui let out a grunt.

Shisui remained calm despite the sudden attack, his hand gripping Kaien's collar. He blinked in astonishment at Kaien's speed, but refrained from striking back.

Kaien's Sharingan flared as he lifted Shisui by the collar and hissed, "Shisui! What's the meaning of operating on that prisoner today? I told you only suitable candidates get used."

Struggling for breath, Shisui met Kaien's glare with shame. "Kaien‑niisan… there weren't enough inmates. He was the only compatible kidney donor."

Kaien's grip loosened, letting Shisui breathe again. He frowned. "Surely we can find a match among all three prison facilities?"

Shisui sighed, exhaustion and frustration in his tone. "The rest are in human experiments or already harvested for organs due to heinous crimes. And ANBU have upped surveillance on Uchiha‐run sections recently."

Suddenly, lights activated in the OR and the door opened. Kaien and Shisui stepped inside as if nothing happened.

"Hehe, thank you, Lord Kaien. I wouldn't have known my back would heal without you," said Inuzuka Yuu, a middle‑aged Inuzuka ninja, supporting his bandaged waist.

Kaien nodded, scanning the scene. "Don't mention it. Inuzuka Yuu, you're a hero of Konoha—serving tirelessly and risking injury for our village. We all owe you."

Yuu's smile grew broader. "We do it for Konoha."

Though the Uchiha clan faced isolation, the Seventh Division—Orochimaru's Phantom Police—enjoyed admiration for its clandestine organ network.

A masked medical ninja approached. "Sir, prescribed medications. Take them regularly. First six months, monthly check‑ups, then quarterly."

Yuu, still weak, smiled. "Much appreciated."

Escorted by hidden operatives, Yuu slipped out a secret exit. Only four remained in the OR.

The surgeon removed his mask: Yakushi Kabuto, the chief surgeon. He bowed. "Replacement kidneys took. Donor subject's kidneys will necrotize. He won't survive more than two months."

Beside him, a second surgeon—revealed only by pale eyes—belonged to the Hyūga clan.

On another bed lay an unconscious inmate, unaware his kidneys had been harvested.

Kaien listened gravely, then nodded. "Keep this discreet. If he dies, make it look natural."

"Understood, sir."

After leaving the prison, Kaien stood under sunlight—and yet, Shisui felt darkness linger.

"Shisui, from now on the Hyūga clan handles prison organ operations. The Uchiha must be ready to withdraw at any sign of trouble."

"Yes, Kaien‑niisan," Shisui replied, respect and resignation in his tone. Since the Nine‑Tails Incident, he'd seen too much darkness.

In recent months, unexplained prisoner deaths rose quietly, but no one noticed. At the same time, critically ill patients spiked among the clans. Repeat offenders served decades, making convenient match pools. Many elite ninjas' old injuries resurfaced, begging for transplants—death‑row inmates provided the organs.

All this relied on Orochimaru's near‑perfect transplant ninjutsu.

"Shisui, we do this not for Orochimaru‑sama," Kaien said softly. "ANBU and Root run similar back‑channel trades. Ours differ by superior tech and supply."

He gestured at Nohara Rin, a Chūnin medical ninja adept at battlefield transplant surgery—proof of Konoha's surgical prowess. Ninjas often faced life‑threatening injuries; organ ninjutsu saved many. But here, they must conceal it, using enemy spies or foreigners as donors—until now, Konoha prisoners gave a broader candidate pool.

Most importantly, ninjas only accept organs from fellow ninjas; civilian grafts always reject.

Shisui had witnessed enough. He nodded. "Kaien‑niisan, I understand."

"No—you don't yet. Do you know why Orochimaru‑sama asked the Uchiha and Hyūga to cooperate in secret?"

Kaien's voice grew solemn. He carried the weight of clan survival. Shisui's potential was too great to ignore—and to let him slip into the Hokage faction unguarded would be a fatal mistake.
